Specified Indemnity Benefits
Our policy provides cash benefits for various accidents and medical situations. Here are some of the covered benefits:
Hospital Care
- Hospital confinement: $150 per day (within 90 days of injury)
- Intensive care: $5,000 per accident
- Ground ambulance: $100 (within 72 hours of injury)
- Air ambulance: $500 (within 72 hours of injury)
Additional Care
- Blood and blood plasma: $150
- Transportation (max 3 trips, over 100 miles from home): $300 per trip
- Family lodging: $100 per day (max lifetime benefit $3,000)
- Recovery after hospital stay: $100 per day
Medical Visits
- Emergency room visit: $100 (within 72 hours of injury)
- Emergency room follow-up: $25 per treatment (up to 4 treatments)
- Urgent Care visit: $100 (within 30 days of injury)
Injuries and Treatments
- Concussion: $100 (within 90 days of injury)
- Physical therapy: $25 per session (max 10 sessions)
- Fractures: $250 for minor, $1,000 for major
Preventive Care
- Health screening test or procedure: $50 (once per year, per insured)
